What is a Home Medication Review? (HMR)
A home medicine review involves a qualified and accredited pharmacist performing home interviews with patients who may be taking multiple, possibly conflicting medications or who may be at risk of not taking their medications correctly.
Having a medication review can reduce the chance of side-effects from medication and ensure that you are taking your medications in such a way as to get maximum benefits.
In order to have a home medication review performed, patients must be recommended by their doctor.
The patient may use a community pharmacist or a pharmacist of their choice.
The pharmacist will visit the patient at their home. Visiting the patient at their home ensures the pharmacist can see how the patient is taking and storing the medications in their every-day environment.
Once the medication review has been performed, the pharmacist will share the report with the patient and their doctor.
The patient will have a follow-up consultation with their doctor to discuss the recommendations of the report.
